This may be due to the student using a different SSN or date of birth on the next award year's FAFSA or a correction to an existing FAFSA. StudentForms uses the name and date of birth when the student creates their original account.  Since the student has changed their SSN or date of birth, StudentForms does not associate the new ISIR with the original ISIR in the system.  


The school user will need to determine which SSN or date of birth is correct for the student.

  • If the ssn and date of birth is correct on the ISIR that is associated with the student's account, the student or school should update the SSN and/or date of birth on the ISIR that is incorrect.  This will allow the subsequent ISIR to be associated with the student's account.
  • If the ssn and date of birth is correct on the ISIR that is not associated with the student's account (student created their account using incorrect DOB), the school support administrator has several options:
    • When the account created has the incorrect DOB, the school user may make a correction to the DOB in StudentForms to update to the correct DOB. Once the corrected ISIR is received, it will update the DOB on the student's account.
      • Please Note: The DOB correction must be made in StudentForms. If a correction is received on the DOB and it is not expected in StudentForms (student-initiated correction, corrections made directly in CPS or corrections from SIS/FAM), it will continue to put the ISIRs into separate accounts.
    • For Single Sign-On (SSO) schools, the school can use the force re-registration button to have the student register their account using the correct DOB.
      • This will require the previous StudentForms transaction to no longer show for the student but a school can still view the transaction by searching for the student.
    • For DSO schools, a support ticket may be opened to have the account switched to use the other ISIR.
      • This will require the previous StudentForms transaction to no longer show for the student but a school can still view the transaction by searching for the student. 


Please Note: Prior to 03/02/2016 the student accounts were matched off of ISIR ID.  Any students who completed their ISIR Prior to 03/02/2016 may still need to be corrected by submitting a support ticket.
