Parent - Account Creation


The first time that a parent is requested to e-sign a document they must create an account. Subsequent documents that require e-signature, the parent may use the same username and password to login. 


For ISIRs, the parent must pass the check with the Social Security Administration on their Name, DOB, and SSN for the student to get the e-sign option. If at least one parent does not pass the check, the student is automatically opted out of e-sign. If the parent does not have a SSN and enters all zeros for the parent SSN on the FAFSA, the check with SSA is not performed and the parent is not eligible for e-signature. If only one parent passed the check with the SSA, the student may only send the e-sign request to the parent who passed the SSA check.


For Cal DREAM ISIRs, the parent is able to have all zeros for their ssn and create an account, as long as they can provide all required information and match to the Cal DREAM ISIR.


Please note: The parent cannot create an account or view documents for e-signature until the student sends the e-signature request.  If the parent is unable to create an account, check the student view tab for the task requiring e-signature to make sure the student has sent the request.  If you see the text "Student has not initiated a signature request.", the student has not sent the request to the parent.


Once the request is sent to the parent without an existing account, the parent clicks on the link provided in the parent e-sign email, the parent is taken directly to the create account screen.




The parent is required to complete the following information:


  • Choose username
  • Create password 
  • Confirm password

More information on username and password requirements are provided below.


The parent is required to confirm their information as it appears on the ISIR (FAFSA) or DREAM ISIR.




  • Parent's first name
  • Parent's last name
  • Parent's date of birth
  • Parent's social security number


The information above must match the ISIR/DREAM ISIR exactly. 

Please Note: The confirm parent information section contains the field labeled Parent's first name. StudentForms accepts both the parent's full first name or the parent's first name initial.  Only the first character entered into the Parent's first name field is used to compare to the FAFSA.


Once all required information is entered, the parent may select the Create Account button.


If all confirm parent matches the ISIR/DREAM ISIR, the parent is taken to the Parent E-sign screen.

Please Note: Prior to July 2nd, 2020 if the parent had multiple students attending the same institution, they were required to create a separate account for each student for whom they needed to e-sign a document. Parents who have multiple accounts, the accounts will not be merged, they will continue to manage their accounts separately.


As of July 2nd, 2020, StudentForms has been updated to allow multiple students to be associated with a single parent account. It requires all students to attend the same institution and the parent information on the ISIR/DREAM ISIR (SSN, DOB, First Initial) matches for all students. StudentForms will automatically link any students whose ISIR/DREAM ISIR matches the parent account information once the e-sign request is sent to the parent. The parent will have a drop-down to toggle between students.


Parent - Username Requirements

A parent's username must be between 8 and 64 characters long. Any of the following may be used:


  • Upper case letters
  • Lower case letters
  • Numbers
  • The following symbols "@", "_", ".", and "-"

The username requirements may also be viewed by selecting the Username requirements link on the create account screen.



Additionally, if the parent enters a username that does not meet the requirements a message appears below the username fields indicating the username requirements.




The parent is also notified if their username has already been used. Usernames must be unique for each account, and may not be associated to multiple accounts.



Parent - Password Requirements

A parent's password must be at least 8 characters long and must contain at least one of each of the following:


  • Upper case letters
  • Lower case letters
  • Number


Special characters are not required but they are permitted to be used in the password.


The password requirements may also be viewed by using the Password requirements link on the create account screen.



If the parent does not enter a password that meets the requirements, a message appears below the field indicating what the password requirements are.



The parent must also confirm the password, if the password and confirm password fields do not match a message is displayed below the field.
